Bankruptcy Kits
Numerous companies sell bankruptcy kits or bankruptcy forms relatively inexpensively, anywhere from $29-100. The kits and forms generally include the Federal bankruptcy forms and may or may not include the local forms that may be needed for your state. It's hard to see what is included online and whether or not the instructions will be clear enough for you to complete the forms sufficiently.
If you are considering one of these kits just beware that when you run into a problem there will be no support, and if your bankruptcy is delayed for any reason and you end up needing the support of an attorney, your costs to fix your bankruptcy petition will be much higher than if you went to an attorney or petition preparer first.
